The Shiley 50XLTCD XLT extended-length cuffed tracheostomy tube is designed to serve patients whose anatomical conditions require a tracheostomy tube longer than standard lengths, particularly when there is a longer-than-usual distance between the stoma and the airway or when tracheal anatomy demands deeper placement. The âdistal extensionâ designation means the extra length is delivered on the distal side (further into the trachea), enabling better positioning in patients with elongated or stenotic tracheas. The high-volume, low-pressure cuff is intended to provide an effective seal during positive-pressure ventilation while minimizing localized pressure on the tracheal mucosa.
The disposable inner cannula with twist-lock retention allows clinicians to remove and replace or clean the cannula without disturbing the outer cannula, which supports easier secretion management and reduces the risk of full tube exchanges. The tip-to-tip radiopaque line makes it easier to confirm correct positioning under imaging (e.g. chest or neck X-ray). The flexible, thermosensitive outer cannula softens at body temperature, helping it conform to the patientâs airway shape, which may reduce irritation or pressure points. The soft, freeâswiveling neck flange facilitates stoma site care, reduces skin pressure, and makes inspection of surrounding skin easier.
In practice, this tube is selected for patients who have âdifficult-to-fitâ anatomy: e.g. patients with longer necks, tracheomalacia, stenosis, or when a standard tube might not reach the desired distal tracheal level. It enables more predictable airway placement without requiring a custom tube.
